About the role
Provides physical therapy services to restore and improve patients' physical, social, and mental functions within a home health and hospice setting.
Responsibilities
- Conducts evaluations
- Develops individualized treatment and discharge plans
- Administers therapy interventions using established protocols
- Supervises assigned assistants
- Completes accurate clinical documentation
- Acts as a co-leader in interdisciplinary patient care teams
- Performs additional duties as assigned
Qualifications
- Doctor of Physical Therapy (DPT) degree or foreign equivalent
- Valid state licensure
- CPR/BLS certification
- Valid driver’s license with minimum age of 21
