Physical Therapist - Home Health
Responsibilities
- Conduct comprehensive in-home physical therapy evaluations, assessing mobility, strength, balance, pain, endurance, and home safety.
- Develop, implement, and regularly update individualized Plans of Care (POC) based on physician orders, patient goals, and measurable outcomes.
- Deliver skilled physical therapy interventions to improve mobility, gait, balance, strength, endurance, and pain management.
- Educate patients and caregivers on diagnosis, safety precautions, fall prevention, home exercise programs, and use of medical equipment.
- Perform home safety assessments and recommend modifications to enhance safety and support independent functioning.
- Maintain accurate, timely, and compliant documentation within the electronic medical record (EMR).
- Collaborate with interdisciplinary team members to coordinate care and optimize patient outcomes.
- Communicate changes in patient condition or barriers to care promptly to physicians and relevant stakeholders.
- Ensure compliance with payer requirements, including documentation standards and eligibility criteria.
- Manage caseload efficiently while meeting scheduling, productivity, and visit timeliness expectations.
Requirements
- HCHB AND OASIS proficient.
- Oregon PT license.
- Current CPR certification.
- Valid driver’s license, auto insurance, and reliable transportation.
- Experience in home health or related clinical settings preferred.

Pay
$52/hr- $56/hr *Pay offered is based on experience, expertise, credentialing, and education.
