Physical Therapist - Home Health

Responsibilities

• Conduct comprehensive in-home physical therapy evaluations, assessing mobility, strength, balance, pain, endurance, and home safety.
• Develop, implement, and regularly update individualized Plans of Care (POC) based on physician orders, patient goals, and measurable outcomes.
• Deliver skilled physical therapy interventions to improve mobility, gait, balance, strength, endurance, and pain management.
• Educate patients and caregivers on diagnosis, safety precautions, fall prevention, home exercise programs, and use of medical equipment.
• Perform home safety assessments and recommend modifications to enhance safety and support independent functioning.
• Maintain accurate, timely, and compliant documentation within the electronic medical record (EMR).
• Collaborate with interdisciplinary team members to coordinate care and optimize patient outcomes.
• Communicate changes in patient condition or barriers to care promptly to physicians and relevant stakeholders.
• Ensure compliance with payer requirements, including documentation standards and eligibility criteria.
• Manage caseload efficiently while meeting scheduling, productivity, and visit timeliness expectations.

Requirements

• HCHB AND OASIS proficient.
• Oregon PT license.
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.
• Experience in home health or related clinical settings preferred.

Pay

$52/hr- $56/hr *Pay offered is based on experience, expertise, credentialing, and education.
