About the Role

We are seeking a Home Health Physical Therapist to provide skilled physical therapy in patients' homes, focusing on individual needs and treatment plans based on physician directives. The role involves patient evaluation, treatment, and professional consultations to promote independence and improve functional outcomes.

Qualifications

• Bachelor's, Master's, or D.P.T. from an accredited Physical Therapy Program
• Oregon Physical Therapist License
• Within 30 days of hire: BLS certification (American Heart Association)
• At least 1 year of clinical experience in home health or healthcare setting, or active enrollment in a postgraduate preceptor program
