Pride Health is actively hiring a Physical Therapist to join their client's Home Health Program in Albuquerque, NM. Coverage areas include a 60-mile radius around Albuquerque, NM. The schedule is Monday - Friday, 8:00 AM - 4:30 PM, for a duration of 6+ months. Local pay is $52.00 - $56.00/hour (W-2) plus mileage reimbursement. There is also a travel pay package of up to $2,700/week gross (including a weekly non-taxable stipend and mileage reimbursement). Negotiable pay is based on experience, expertise, credentialing, and education.
About the Role
The Physical Therapist will perform comprehensive physical therapy evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risks. They will develop, implement, and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management, and functional independence. The therapist will deliver skilled interventions such as therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety education. They will educate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation techniques. Accurate and timely documentation (evaluations, visit notes, progress reports, and discharge summaries) using EMR systems is required. The therapist will coordinate care with physicians and interdisciplinary team members, including nurses, occupational therapists, speech therapists, and home health aides. Communication of changes in patient condition, safety concerns, or barriers to progress must be done promptly to the care team. Participation in case conferences, care coordination activities, and team collaboration is expected. Compliance with CMS regulations, infection control standards, and agency policies is necessary. Efficient management of a mobile caseload while meeting productivity and documentation expectations is required. Adaptability to new environments, workflows, and patient populations as a travel clinician is essential.
Required Qualification/Certification
- Graduate of an accredited Physical Therapy program
- Active Physical Therapist license in the state of assignment
- Home Health experience required
- Willingness to travel within the assigned coverage area
- Current BLS certification
- Valid driver’s license
