Physical Therapist (Home Health)

About the role

Provides physical therapy services to improve patients' physical, social, and mental functions through personalized treatment programs. Acts as a co-leader within an interdisciplinary team to promote patient recovery and well-being.

Responsibilities

• Conducts thorough evaluations to develop individualized treatment plans.
• Implements therapy interventions using appropriate techniques and protocols.
• Communicates care and discharge plans effectively to patients and team members.
• Maintains accurate clinical documentation within specified deadlines.
• Supervises assigned therapy assistants and performs additional related duties as needed.

Qualifications & Skills

• Doctor of Physical Therapy (DPT) degree, with licensure in applicable states.
• Valid physical therapy license and CPR/BLS certification.
• Valid driver’s license and minimum age of 21.
• Strong clinical assessment, treatment planning, and documentation skills.
• Ability to work collaboratively within an interdisciplinary team.
