About the role

Companion Home Health is an Oklahoma-owned, Medicare-certified agency providing skilled therapy in patients' homes across central Oklahoma.

Responsibilities

  • Provide skilled physical therapy according to the physician’s plan of treatment
  • Absorb only clients referred by the Director of Nursing or designee
  • Assess function, apply appropriate tests, and help develop and update the plan of care
  • Treat patients to relieve pain, restore function, and maintain the highest possible performance using the treatments and modalities in the plan of care
  • Re-evaluate regularly, revise the plan of care as needed, and document why a identified problem is not addressed when that is the case
  • Start appropriate diagnostic, preventive, and rehabilitative procedures and refer to other disciplines when the patient's needs require it
  • Observe changes in condition and report them promptly to the physician and/or RN/Case Manager
  • Teach patients and families home exercise programs, safety, use of prosthetic/orthotic devices, and durable medical equipment
  • Cook up care with nursing and other disciplines and attend care conferences as needed
  • Complete accurate documentation within 24 hours of the visit
  • Participate in quality improvement, in-services, and ongoing professional education
  • Provide staff education when requested and arranged by the RN/Case Manager
  • Schedule and work setting

Requirements

  • Graduate of a physical therapy program approved by CAPTE / APTA (or equivalent approved program)
  • Current Oklahoma Physical Therapist license
  • Current CPR
  • Valid driver’s license
  • Ability to work independently and as part of an interdisciplinary team
  • Strong verbal, written, and interpersonal communication skills
  • Ability to work a flexible schedule

Physical requirements

  • This role requires travel between homes and the ability to provide hands-on therapy in varied home settings.
  • That typically includes standing, walking, stooping, kneeling, reaching, and assisting patients.
  • You may lift, carry, push, or pull about 10-15 lbs. intermittently.
