About the role

Acclaim Home Health seeks a skilled, compassionate Physical Therapist to join our growing team in Worcester, MA. As a home health PT, you'll have the opportunity to make a meaningful impact by helping patients improve mobility, regain strength, increase independence, and safely remain in their homes.

Responsibilities

• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide therapeutic interventions to improve strength, mobility, balance, coordination, and functional independence.
• Help patients safely achieve their mobility and functional goals in their home environment.
• Educate patients and caregivers on exercises, mobility strategies, fall prevention, and appropriate use of equipment.
• Collaborate with patients, families, physicians, and interdisciplinary team members to coordinate care and achieve optimal outcomes.
• Monitor patient progress and adjust treatment plans as needed.
• Complete timely and accurate clinical documentation in accordance with company and regulatory requirements.
• Maintain current clinical knowledge through continuing education and professional development.

Requirements

• Current Massachusetts Physical Therapist license.
• Home health or outpatient experience preferred.
• Valid driver's license and reliable transportation.
