Overview
Soliciting a licensed Physical Therapist to deliver personalized 1:1 rehabilitative therapy in patients' homes and communities.
Responsibilities
- Conducting comprehensive assessments
- Developing treatment plans
- Delivering therapy
- Adjusting care based on patient progress
- Practicing independently within an interdisciplinary team
- Focusing on patient-centered outcomes
Qualifications
- New York State License and current registration in Physical Therapy
- Bachelor's Degree in Physical Therapy from an approved program
- Minimum of one year of physical therapy experience or successful completion of a Rehab Intern probationary period
- Basic PC skills; bilingual abilities may be required based on operational needs
