Physical Therapist - Home Care

About the Role

As a Physical Therapist, you will provide personalized 1:1 rehabilitative therapy to patients in their homes or care facilities, helping them recover from surgery, illness, or injury.

Responsibilities

• Conduct comprehensive patient assessments to determine needs and develop treatment plans
• Deliver tailored therapy programs and adjust plans based on patient progress
• Practice independently within an interdisciplinary care team

Requirements

• Minimum of one-year physical therapy experience or successful completion of a rehab intern probationary period
• Active Physical Therapist licensure in New York State
• Valid driver’s license required outside Manhattan

Pay

USD $85,000 - $106,300 annually, with pay per visit varying by location and day.
