Physical Therapist- Home Care
Employment Status: Full time, Day shift. Facility: 1615 Franklin Rd - Roanoke. Mileage reimbursement, company equipment, and sign-on bonus eligible.
About the Role
This is an opening for our Home Health Float Team, based out of Roanoke, but providing coverage to surrounding areas as well. The Physical Therapist (PT) confirms and interprets physician referrals, performs patient assessments, and plans and implements individualized treatment programs considering the patient's developmental and/or chronological age and medical diagnoses in accordance with regulations governing the principles and practice of Physical Therapy.
You will provide patient care based on physician referral and age-appropriate assessment, coordinate care through communication with patients, families, therapists, clinical team leaders, managers, physicians, and other healthcare team members. You will document in patient medical records and complete other patient care-related forms in compliance with regulatory agencies and department/hospital policies.
Supervises Physical Therapist Assistants (PTAs) and collaborates with other therapy staff, support staff, volunteers, and students to achieve efficient clinical activity and quality patient care. Participates in patient care and departmental/professional activities such as in-services, performance improvement, program development, new employee orientation, committee membership, and peer review.
Work is performed primarily indoors in clinical settings, including hospitals, nursing homes, outpatient sites, or patient homes. May require working at other Carilion facilities to meet staffing needs. Home care practitioners must provide their own transportation and travel daily between patient homes.
Responsibilities
- Confirm and interpret physician referrals.
- Perform patient assessments and plan individualized treatment programs considering patient age and medical diagnoses.
- Provide patient care based on physician referral and age-appropriate assessment.
- Coordinate patient care and clinical activities through communication with patients, families, therapists, and healthcare team members.
- Document in patient medical records and complete other patient care-related forms.
- Supervise PTAs and collaborate with therapy staff, support staff, volunteers, and students.
- Participate in in-services, performance improvement, program development, and other departmental activities.
- Consistently deliver excellent customer service and patient care.
- Orient new employees and provide guidance to volunteers, students, and support staff.
- Suggest cost containment initiatives in your work unit/department.
Requirements
- Graduate of an approved school of Physical Therapy.
- Bachelor's degree as a Physical Therapist and 5 years of experience, OR Master's or Doctorate degree required.
- One year of clinical experience required for Home Health positions.
- Licensed to practice in the Commonwealth of Virginia by the Virginia Board of Physical Therapy.
- AHA BLS-certified (from a Carilion approved program) within six months of employment.
- Effective interpersonal, communication, customer service, and teamwork skills.
Skills
- Knowledge of principles of life span growth and development.
- Ability to assess data reflective of patient status and interpret appropriate information for age-specific needs.
- Physical ability to lift, manipulate, push, pull, bend, and walk with patients, including those weighing over 200 pounds.
- Occasional exposure to serious injury and frequent environmental hazards, including blood and bodily fluids, requiring standard precautions.
Pay
Compensation Range: $39.00 - $62.40 per hour. Employee pay is based on a variety of factors, including qualifications, credentials, education, and prior work experience. The compensation range does not include benefits or pension plan.
