Overview

We are seeking a Physical Therapist to provide personalized, independent rehabilitative care in patients' homes and communities.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Deliver one-on-one therapy sessions tailored to patient needs.
• Monitor progress and adjust care plans accordingly.
• Collaborate within an interdisciplinary team to ensure optimal patient outcomes.

Qualifications

• Minimum of one-year physical therapy experience or successful completion of probation as a Rehab Intern.
• Licensure as a Physical Therapist in New York State.
• Valid driver's license for areas outside Manhattan.
