Physical Therapist- Home Care
About the role
This position is for a designated area therapist to cover primarily in the Roanoke region. The Physical Therapist (PT) confirms and interprets physician referrals, performs patient assessments and plans and implements individualized treatment programs taking into account the patient's developmental and/or chronological age and medical diagnoses in accordance with the regulations governing the principles and practice of Physical Therapy.
Responsibilities
- Transform healthcare by confirming and interpreting physician referrals, performing patient assessments and implementing individualized treatment programs.
- Provide patient care based upon physician referral and age-appropriate assessment, taking into consideration the patient’s developmental/chronological age and medical diagnoses.
- Clinically coordinate patient care and clinical activities through written and/or verbal communication with patients, families, therapists, Clinical Team Leaders, managers, physicians and other members of the health care team.
- Document in the patient’s medical record and complete other patient care-related forms in compliance with regulatory agencies and department/hospital policies and procedures.
- Supervise Physical Therapist Assistants (PTAs) as provided by State of Virginia licensure/practice standards and Federal regulations.
- Suggest cost containment initiatives in immediate work unit/department.
- Interpret physician orders, perform patient assessments, and plan and implement individualized patient treatment programs.
Requirements
- Education: Graduate of approved school of Physical Therapy. Bachelor's degree as Physical Therapist and 5 years of experience, OR Master's or Doctorate degree required.
- Licensure/Certification: Licensed to practice in the Commonwealth of Virginia by the Virginia Board of Physical Therapy. AHA BLS-certified (from a Carilion approved program) within six months of employment.
Qualifications
- Experience: Experience in site specific level of care preferred but not required. For Home Health positions one year of clinical experience required.
- Effective interpersonal, communication, customer service and teamwork skills required.
Skills
- Knowledge of the principles of life span growth and development.
- Ability to interpret appropriate information needed to identify each patient’s requirements relative to their age specific needs.
