Physical Therapist - Home Care

About the role

Seeking a licensed Physical Therapist to provide personalized, 1:1 rehabilitative therapy in patients' homes and communities. The role involves conducting comprehensive assessments, developing treatment plans, and adjusting care based on evolving patient needs. You will work independently within an interdisciplinary team, delivering therapy to support recovery from surgery, illness, or injury.

Responsibilities

• Practice independently in the community.
• Assess patient needs and develop tailored treatment plans.
• Deliver rehabilitation therapy in home or care settings.
• Evaluate and modify care plans as patient needs change.

Requirements

• New York State License and registration in Physical Therapy.
• Bachelor’s Degree in Physical Therapy from an approved program.
• Minimum one year of physical therapy experience or successful completion of a rehab intern program.
• Basic PC skills; bilingual abilities may be preferred.
