Physical Therapist - Home Care

About the role

We are seeking Physical Therapists to provide personalized, home-based rehabilitative care for patients recovering from surgery, illness, or accidents.

Responsibilities

• Practice independently within an interdisciplinary care team
• Conduct patient assessments and develop tailored treatment plans
• Deliver one-on-one therapy sessions in patients' homes
• Monitor progress and adjust care plans accordingly
• Educate patients and caregivers to promote recovery and independence
• Collaborate with physicians and team members
• Utilize innovative technology to streamline documentation

Requirements

• At least one year of Physical Therapy experience preferred or completion of a rehab intern program
• Current NY State Physical Therapist License and Registration
• Valid driver’s license may be required based on territory
