About the role
We are seeking a Home Health Physical Therapist to deliver personalized care in patients' homes, helping them regain strength, mobility, and independence.
Responsibilities
- Plan and administer physical therapy treatments
- Assess and document patients' physical status
- Develop rehabilitation programs aligned with physician plans
- Supervise PTAs and aides
- Report patient progress to the care team
- Provide clear care instructions
- Collaborate with the care team and document patient progress
Qualifications
- Current, unrestricted PT license
- CPR certification
- Experience working with healthcare professionals
- One year of PT experience (preferred)
