Become a part of our caring community NOT Remote - Relocation Assistance for Milwaukee Opening This fulltime PT would be working in Milwaukee Wisconsin. As a fulltime therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility and independence, from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you'll empower individuals to overcome limitations and enjoy a better quality of life.

Responsibilities

• Sign On/Relocation Offered
• Deliver personalized care to patients in Washington, Ozaukee Counties, Wisconsin (Flexible, Territory-Based Role)
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals
• Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation
• Document patient observations, interventions, and evaluations promptly and thoroughly
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries

Requirements

• Use your skills to make an impact
• Have a degree from an accredited Physical Therapy Program (approved by the APTA)
• Have a current and unrestricted Physical Therapy license
• Have a current CPR certification
• Have one year of experience as a Physical Therapist (preferred)

Pay

Pay Range $59.00 - $83.00 pay per visit/unit $93,000 - $128,000 per year base pay
