Physical Therapist, Home Visits

About the role

Become a part of our caring community in Milwaukee, Wisconsin. This full-time position offers personalized care to patients in Washington and Ozaukee counties, providing them with the tools to regain strength, mobility, and independence from the comfort of their homes.

Responsibilities

• Deliver personalized care that focuses on functional improvement
• Achieve the highest level of physical function for patients
• Assess and document patients' physical strengths
• Develop and implement a rehabilitation program consistent with physician’s Plan of Treatment
• Provide clear written care instructions to PTAs and Home Health Aides
• Oversee the performance and documentation of PTAs and Home Health Aides
• Report patient progress to the physician, Clinical Manager, care team, patient, and family
• Submit all required evaluations, plans, progress notes, and discharge summaries

Requirements

• Accredited Physical Therapy Program (approved by APTA)
• Current and unrestricted Physical Therapy license
• CPR certification
• One year of experience as a Physical Therapist (preferred)

Qualifications

• Strong interpersonal and communication skills
• Ability to work independently and as part of a team
• Knowledge of home health care protocols and procedures
• Flexibility to travel to Humana's offices for training or meetings

Skills

• Excellent clinical judgment and problem-solving skills
• Effective communication and collaboration abilities
• Proficiency in Microsoft Office applications

$59.00 - $83.00 pay per visit/unit
$93,000 - $128,000 per year base pay
