Physical Therapist/Full time/Part-time/Per Diem

Responsibilities

• Act in the capacity of a professional practitioner to provide diagnostic, palliative, preventive, or rehabilitative/habilitative services to eligible persons.
• Provide services in accordance with generally acceptable community standards of professional practice and in conformance to the requirements of all-applicable statutes and regulations.
• This includes but is not limited to 14 NYCRR Parts 679, 620, 624, 633, 635, and 636.
• Document in each person’s confidential clinical record all diagnostic assessment reports, individual plans of treatment, and treatment notes regarding service being recommended and/or delivered.
• Per treatment recommendations, consult with each person receiving services and/or collaterals regarding treatment outcomes and course of treatment.
• Serve as clinical supervisor to physical therapist assistants as needed.
• Review and sign off as necessary the written work of PTA staff.
• Serve as a resource to all staff and consumers on a formal and informal basis.
• Develop a working knowledge of relevant OPWDD and SARC policies and procedures.
• Attend in-service training, and other professional development as required.

Requirements

• Appropriate education from an accredited institution and current NYS licensure in physical therapy.
• Valid NYS driver’s license that meets Agency guidelines.
• Basic computer skills.
• One-year full-time treatment experience with persons having developmental disabilities preferred but not required.

Pay

$33.00-37.88 per hour.

Schedule

Full time (35 hours/week), Part time (20 hours/week) or Per Diem (FLEXIBLE). No weekends!
