Physical Therapist-Part Time/Per Diem

Major Responsibilities

• Home visitation with infants, toddlers, and their families
• Aid families in implementing strategies to enhance their child’s development
• Create and implement Individualized Family Service Plans (IFSPs) with families
• Coordinate services for families transitioning to preschool programming
• Collaborate with a trans-disciplinary team of early childhood professionals
• Perform other duties as assigned

Knowledge, Skills, And Abilities

• Proficient in Microsoft Office, including Excel
• Strong interpersonal skills
• Effective relationship-building abilities
• Excellent organizational skills and computer literacy

Qualifications/Requirements

• CT DPH licensed/certified Physical Therapist
• Experience with infants, toddlers, and families
• Completion of service coordination training or willingness to complete training
• Excellent organizational skills and computer literacy

Physical Requirements

• Frequent standing, bending, kneeling, and therapeutic holding
• Must be able to perform essential functions with or without reasonable accommodations

Travel

• Travel required within the Torrington area
• Requires reliable transportation, a valid driver’s license, and car insurance
