We are seeking a full-time Physical Therapist for a home health, field-based role aimed at helping patients improve mobility, manage long-term conditions, and recover from injuries. The role offers independent work with opportunities to make a measurable impact on patient outcomes. It involves driving between patient locations and working within a supportive team with over 30 years of community care experience.

Responsibilities

• Providing patient-centered physical therapy services in home settings
• Assessing patient needs and developing personalized treatment plans
• Monitoring progress and adjusting interventions accordingly
• Ensuring compliance with safety and health regulations

Requirements

• Degree in Physical Therapy (Bachelor's, Master’s, or Doctoral) from an accredited program
• Active State License
• BLS Certification (may be required)
• Strong communication, independence, and time management skills

Pay

Competitive per-visit pay ($50–$100+)
