About the Role
We are seeking a licensed Physical Therapist for a full-time role in a Skilled Nursing Facility or Long-Term Care setting.
Responsibilities
- Evaluating and treating patients with mobility, strength, balance, and functional impairments
- Creating and implementing personalized care plans
- Collaborating with team members to ensure quality outcomes
- Maintaining documentation to meet regulatory requirements
- Ensuring physician orders are current and properly obtained
Qualifications
- Degree from an APTA-accredited Physical Therapy program (BS, MS, or DPT)
- Current license as a Physical Therapist in the practice state
- Strong communication and organizational skills
