Physical Therapist - Full-Time

About the role

Seeking licensed Physical Therapists to provide personalized in-home and senior living care, enhancing mobility and independence for older adults. Deliver one-on-one physical therapy sessions in private residences and senior communities.

Responsibilities

• Perform evaluations, develop individualized care plans, and implement evidence-based treatments.
• Support patient wellness and functional independence through therapy.
• Document patient progress accurately within electronic medical records.
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Utilize telehealth and virtual care to extend access and flexibility.

Qualifications

• Valid physical therapy license or eligibility in practicing state.
• Degree from an accredited program.
• Proficiency in computer and EMR systems.
• Current CPR certification.

Physical Requirements

• Ability to exert medium effort, lift up to 50 lbs, and assist with patient transfers.
• Perform CPR and reposition patients as needed.
• Drive to patient locations and wear PPE when required.
