Physical Therapist - Full-Time

About the Role

A licensed Physical Therapist delivers one-on-one physical therapy to older adults in private residences and senior living communities, focusing on improving mobility, independence, and quality of life through evidence-based treatments within a flexible, patient-centered model.

Responsibilities

• Perform evaluations, develop individualized care plans, and deliver therapy sessions supporting wellness and functional independence.
• Document patient progress accurately in electronic medical records.
• Collaborate with an interdisciplinary team including PTs, OTs, and SLPs.
• Educate patients, caregivers, and staff on safety and aging in place.
• Utilize telehealth and virtual care to enhance access and continuity.

Requirements

• Valid state licensure or eligibility.
• Degree from an accredited Physical Therapy program.
• Proficiency in EMR and basic computer skills.
• Current CPR certification.
• Physical capability to perform transfers, reposition patients, and perform therapeutic activities, including lifting and mobility tasks.
• Must meet physical demands of in-home and community care.
