About the Role

Responsible for coordinating and delivering physical therapy services, developing individualized treatment plans, and implementing therapeutic activities to promote patient health and recovery.

Responsibilities

• Evaluate patients through various sources, including records and interviews.
• Design and modify personalized treatment programs based on ongoing assessments.
• Provide direct therapy in different settings, aligning with medical and rehabilitation plans.
• Prepare documentation such as evaluations, treatment plans, and progress notes.
• Communicate effectively with patients, families, physicians, and team members.
• Collaborate within interdisciplinary teams to ensure comprehensive care.
• Participate in ongoing professional development activities.

Requirements

• Active state license as a Physical Therapist.
• Effective communication and visual acuity; ability to work as part of a team.
• Preferred 1-3 years of licensed physical therapy experience.
