About the Role
Licensed Physical Therapist focused on providing personalized, one-on-one care to older adults in private residences and senior living communities, utilizing a Geriatric House Calls™ model.
Responsibilities
- Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy aimed at improving mobility, independence, and overall wellness.
- Document patient progress accurately within electronic medical records, adhering to regulatory standards.
- Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and staff to promote safety and aging in place.
- Incorporate telehealth and virtual care to extend access and flexibility.
Qualifications & Skills
- Valid Physical Therapy license and degree from an accredited program.
- Proficiency with computers and EMR systems.
- Current CPR certification.
- Ability to meet physical demands, including patient transfer assistance, lifting, repositioning, and driving between locations.
