Physical Therapist - Full Time

About the role

The Physical Therapist at Hackensack Meridian Health is responsible for providing comprehensive, individualized, and evidence-based physical therapy services. This includes preparing for evaluations, administering tests, formulating treatment plans, collaborating with interdisciplinary teams, and documenting patient progress.

Responsibilities

• Prepares for evaluations, obtains patient history, and performs systems reviews.
• Selects and administers tests and measures, synthesizes evaluation results, and formulates diagnosis, goals, prognosis, and plan of care.
• Establishes and revises treatment plans based on patient assessments and collaborates with interdisciplinary teams.
• Provides patient/client instruction, interventions, and supports referrals as needed.
• Collects and records patient care data, prepares billing documents, and adheres to regulatory requirements.
• Supervises other personnel as necessary.

Qualifications

• Graduate of a CAPTE accredited Physical Therapy Program or equivalent.
• Excellent written and verbal communication skills.
• Proficient in computer skills, including Microsoft Office and/or Google Suite platforms.
• Minimum of 1 year of experience preferred.
• Physical Therapist License required.
• BLS AHA Basic Health Care Life Support certification required.
• Specialty certifications preferred.
