About the Role

Provides outpatient physical therapy services in patients' homes, evaluating, planning, treating, and implementing evidence-based interventions in accordance with professional standards and organizational policies.

Responsibilities

  • Conduct therapy evaluations, develop individualized treatment plans, and manage full patient caseloads.
  • Administer treatments, educate patients and caregivers, and coordinate discharge planning.
  • Handle documentation, billing, scheduling, and administrative duties.
  • Communicate professionally, supervise support staff, and collaborate with multidisciplinary teams.
  • Coordinate with healthcare leaders and referring physicians to ensure cohesive patient care.
  • Adhere to organizational policies, ethical standards, and scope of practice.
  • Flexibility to serve in other departments based on patient needs; may serve as a clinical instructor and participate in committees.

Requirements

  • Current state licensure in physical therapy.
  • Graduate from an accredited physical therapy program (Bachelor’s, Master’s, or Doctorate).
  • Homecare driving requirements and BLS certification or ability to obtain within a month.

Schedule

Generally weekdays, 8:00am - 4:30pm, with flexible options; no weekends, holidays, or on-call.
