Physical Therapist - Full Time

About the Role

A Physical Therapist provides comprehensive therapy services in a skilled nursing facility, focusing on improving residents' mobility, strength, balance, and independence.

Responsibilities

• Provide therapy services to adult and elderly residents
• Develop and execute personalized treatment plans
• Support residents in achieving rehabilitation milestones
• Maintain accurate documentation and comply with standards
• Collaborate with therapy and nursing teams
• Educate residents and families on safety and independence techniques
• Participate in team meetings and care planning

Requirements

• Degree in Physical Therapy from an accredited program
• Current Wisconsin Physical Therapist license
• Experience with adult and geriatric populations, preferably in long-term care
• Authorization to work in the U.S.
• BLS certification may be required

• Clinical documentation skills; familiarity with Net Health is advantageous
