Pay

Pay range: $41.04/hour to $52.00/hour (hourly non-exempt based on qualifications, experience, and location)

Home Branch

Branches available: Alexandria, MN OR Baxter, MN.
Position will float across the Vivie continuum.

Key Responsibilities

• Play a key role in providing flexible, high-quality clinical coverage for home health clients
• Deliver skilled physical therapy and case management as needed
• Evaluate clients by obtaining and interpreting subjective and objective data
• Develop personalized treatment plans with short-term and long-term goals in accordance with physician orders
• Maintain accurate client records and billing
• Document all services accurately and promptly using Vivie’s EMR system
• Demonstrate a thorough understanding of all payer sources and authorization requirements
• Complete and document supervisory visits with PTAs and/or HHAs in compliance with regulatory guidelines
• Serve as primary client coverage for staff members on FMLA, sick leave, vacation, or during open positions
• Manage cases as needed based on duration of coverage, including all visit types and coordination with the interdisciplinary team
• Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team
• Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery

Qualifications

• Current Physical Therapist license in the state of Minnesota
• CPR certification required within 6 months of hire
• Willingness to travel across the region as necessary
• Valid driver’s license and access to an insured personal vehicle
• Ability to pass state mandated background check
• Ability to read, write, and speak English proficiently to communicate effectively with clients, families, and team members
• Physical capability to perform job duties
