Physical Therapist - Digital Health Specialist - Hybrid Role

About the Role

Provide outpatient physical therapy services, including orthopedic, neurologic, and neuromuscular care, through in-person and telehealth platforms. Serve as a liaison between patients, in-clinic providers, and the Digital Health team to optimize remote service delivery and growth.

Responsibilities

• Develop personalized treatment plans, monitor patient outcomes using advanced software, and document evaluations, treatments, and progress notes.
• Deliver telehealth evaluations, assessments, and treatment plans, including Remote Therapeutic Monitoring (RTM) and virtual care services.
• Support program development, marketing efforts, and maintain regional performance metrics through regular reporting and data analysis.

Requirements

• Diploma from a CAPTE-accredited Physical Therapy Program.
• Florida state licensure (or in process).
• Minimum 2 years of outpatient clinical experience; digital health experience preferred.
• Fluent in English and Spanish.
