Physical Therapist Case Manager - Tampa

Responsibilities

• Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of Physical Therapy.
• Provides the appropriate level of clinical direction for Physical Therapy Assistants.
• Plans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patient's needs are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.
• Manages the activities of the multi-disciplinary team providing care to the patient.
• Maintains ongoing communication with all disciplines regarding the patient's progress and care plan.
• Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.
• Interprets agency policy to patients/families.
• Reviews patients visits weekly and provides plan to PCS and team assistant, including anticipated weekend visits.
• Informs team assistant of any schedule changes and communicates caseload and patient care issues to Patient Care Supervisor.

Requirements

• BLS (Basic Life Support) certification.
• PT (Physical Therapist) license.
• Driver's License from the State of Florida (required within 30 days of hire).

Qualifications

• Bachelor's degree in Physical Therapy.
• Prior experience in physical therapy, preferably in home care setting.
• One year of relevant experience in physical therapy.

Compensation is commensurate with experience.

Schedule

Full-time, exempt position. Shift hours: 8:00 AM - 5:00 PM. Occasional weekend work.
