Physical Therapist, Care Team Coordinator, Home Care Therapy

About the role

As The Care Team Coordinator (CTC), you will be responsible for a client care team at Main Line Health Home Care. You will be responsible for the coordination of clinical care across the disciplines and health care providers. You will also work in conjunction with the Operations Manager to develop and monitor client care services based on customer needs and oversee the day to day operation of the care team.

Responsibilities

Qualifications

• Minimum 3 years of physical therapy clinical experience in a home care position
• Minimum 2 years of management/leadership experience preferred
• Bachelor's degree in allied health field required; master's degree preferred
• 3-5 years recent clinical experience in an allied health profession or home care required
• Management/Supervisory experience preferred

Licensures & Certifications

• Licensed in Commonwealth of PA in clinical profession required (PA Physical Therapist License)
• Provider BLS (American Heart Assoc/American Red Cross) required
