About the role
VNA & Hospice of the Southwest Region of Rutland, VT is looking to hire a Physical Therapist (PT) to provide home health therapy. This position offers a competitive wage and up to a $10,000 sign-on bonus, along with a supportive work environment.
Responsibilities
- Evaluate, develop, implement, and document a plan of treatment for patients of all ages.
- Serve as a case manager providing supervision of students, physical therapist assistants (PTA), and home care aides.
- Monitor and amend patient visit schedules to maximize productivity and patient need.
- Perform comprehensive (OASIS and PT) assessments of assigned patients.
- Develop a plan of care to promote optimal functional independence and safety.
- Provide skilled physical therapy treatment including therapeutic techniques and patient/caregiver education.
- Assess patient status and progress on each visit.
- Coordinate patient care with other clinical team members.
- Supervise and direct clinical activities of PTAs, PT students, and home care aides.
- Complete and coordinate required paperwork.
Qualifications
- Bachelor's degree in Physical Therapy from an accredited university or college required.
- Master's degree preferred.
- 2+ years of PT experience preferred.
- Valid State Driver's License required.
