Physical Therapist

About the role

VNA & Hospice of the Southwest Region of Rutland, VT is looking to hire a Physical Therapist (PT) to provide home health therapy. This position offers a competitive wage and up to a $10,000 sign-on bonus, along with a supportive work environment.

Responsibilities

• Evaluate, develop, implement, and document a plan of treatment for patients of all ages.
• Serve as a case manager providing supervision of students, physical therapist assistants (PTA), and home care aides.
• Monitor and amend patient visit schedules to maximize productivity and patient need.
• Perform comprehensive (OASIS and PT) assessments of assigned patients.
• Develop a plan of care to promote optimal functional independence and safety.
• Provide skilled physical therapy treatment including therapeutic techniques and patient/caregiver education.
• Assess patient status and progress on each visit.
• Coordinate patient care with other clinical team members.
• Supervise and direct clinical activities of PTAs, PT students, and home care aides.
• Complete and coordinate required paperwork.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited university or college required.
• Master's degree preferred.
• 2+ years of PT experience preferred.
• Valid State Driver's License required.
