Physical Therapist focused on providing personalized, one-on-one care to older adults in private residences and senior living communities.

Responsibilities

• Conduct evaluations and develop individualized treatment plans
• Deliver evidence-based therapy to improve mobility and independence
• Document care in EMR
• Collaborate with interdisciplinary teams
• Educate patients and caregivers
• Incorporate telehealth when appropriate

Qualifications & Skills

• Valid physical therapy license
• Degree from an accredited program
• Proficiency in EMR and basic computer skills
• Current CPR certification
• Ability to meet physical demands such as lifting, repositioning patients, and driving

Key Duties

• Support patient wellness and promote safety
• Deliver therapy sessions across various settings
• Support aging in place through education and virtual care
