About the role

We are seeking a licensed Physical Therapist to provide one-on-one in-home and senior living care to older adults, supporting mobility, independence, and quality of life.

Responsibilities

• Conduct evaluations
• Develop individualized treatment plans
• Deliver evidence-based therapy
• Document in EMR
• Collaborate with an interdisciplinary team
• Educate patients and caregivers

Qualifications

• Valid PT license
• Degree from an accredited program
• Proficiency with EMR and computers
• CPR certification

Physical demands

• Lifting
• Repositioning patients
• Extended standing/walking
• Driving to patient locations
