Job Summary
Provides physical therapy evaluation, management, and treatment services to patients referred by physician departments. Services are consistent with the department's mission statement and responsive to unique patient needs.
Essential Responsibilities
- Greets patient, establishes rapport, and orients patient to physical therapy.
- Conducts initial evaluation, including interpreting referral information, reviewing past history, documenting patient concerns, performing an examination, initiating exercises or equipment, establishing a treatment plan, and discharge planning.
- Develops treatment plan with clinical guidelines, discussing it with the patient/family and referring physician.
- Manages case as patient progresses, delegates tasks to clinical staff, presents cases at conferences, provides therapy consistent with examination, evaluation, diagnosis, prognosis, and plan, reviews documentation, revises plan as needed, and communicates changes to customers and staff.
- Establishes discharge plan and documents it, communicating it to customers and staff.
- Participates in program development and participates in problem-solving initiatives.
Basic Qualifications
- Bachelor's degree in Physical Therapy from an accredited college or university.
- CPR/AED for Professional Rescuers required at hire or Basic Life Support required at hire.
- Physical Therapist License (Hawaii) required at hire.
Preferred Qualifications
- One year of related experience in acute care, skilled care facility, sub acute care, or outpatient settings.
- Doctorate in Physical Therapy.
