About the role
Licensed Physical Therapist providing in-home and senior living care to older adults, utilizing the Geriatric House Calls™ model.
Responsibilities
- Conduct evaluations and develop individualized treatment plans
- Deliver evidence-based therapy to improve mobility and independence
- Document care in EMR
- Collaborate with interdisciplinary teams
- Educate patients and caregivers
- Incorporate telehealth when appropriate
Qualifications & Skills
- Valid state license
- Degree from an accredited physical therapy program
- Proficiency in EMR and computers
- Current CPR certification
- Physical ability to perform patient transfers, repositioning, and travel
