Responsibilities
- Conduct evaluations and develop treatment plans.
- Deliver evidence-based therapy to improve mobility and independence.
- Document care in EMR.
- Collaborate with interdisciplinary teams.
- Educate patients and caregivers.
- Incorporate telehealth when appropriate.
Qualifications
- Valid Physical Therapy license.
- Degree from an accredited Physical Therapy program.
- Proficiency in computer and EMR systems.
- Current CPR certification.
Skills & Requirements
- Ability to meet physical demands of in-home care, including lifting, repositioning, and assisting with transfers.
- Reliable transportation to patient locations.
- Wear required PPE as needed.
