Physical Therapist

About the role

This position involves providing one-on-one physical therapy to older adults in private homes and senior living communities, supporting mobility, independence, and quality of life through evidence-based treatments.

Responsibilities

• Perform evaluations, develop individualized care plans, and deliver therapy sessions
• Document care in EMR systems per regulatory standards
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff on safety and aging in place
• Incorporate telehealth and virtual care to enhance access and flexibility

Qualifications & Skills

• Valid Physical Therapy license
• Degree from an accredited program
• Proficiency with EMR systems
• Current CPR certification
• Ability to meet physical demands such as lifting, repositioning patients, and driving between locations
