100% FTE, day shift role. Monday–Friday, 8:00 AM–5:00 PM. Position is located at the Headache Clinic in Rockford. A $20,000 sign-on bonus may be available.

Qualifications
- Graduate of an accredited Physical Therapy program (or foreign equivalent as determined by the State of Illinois Department of Professional Regulation)
- Participates in educational programs on an ongoing basis and completes mandatory continuing education units (CEUs)
- Current license from the State of Illinois or in the process of acquiring licensure
- Applicants must present acceptable evidence of licensure or authorization to test at the time of employment
- New employees will not be allowed to work until evidence of licensure eligibility is provided
- CPR certification
