Physical Therapist

About the Role

Seeking licensed Physical Therapists to provide personalized, in-home and senior living care, including virtual sessions, to older adults.

Responsibilities

• Deliver one-on-one physical therapy in private homes and senior communities
• Conduct evaluations, develop individualized treatment plans, and implement evidence-based therapies
• Support patient mobility, independence, and wellness
• Document care accurately within electronic medical records
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Utilize telehealth and virtual care to enhance access and flexibility

Requirements

• Valid Physical Therapy license or eligibility in practice state
• Degree from an accredited PT program
• Proficiency in computer use and EMR systems
• Current CPR certification

Physical Requirements

• Ability to lift 10–50 lbs and assist with patient transfers
• Perform CPR and moderate physical activities, including standing, walking, and climbing stairs
• Drive between patient locations and wear PPE as needed
