About the role

Seeking licensed Physical Therapists to deliver personalized, one-on-one care to older adults in their homes or senior living communities, utilizing the Geriatric House Calls™ model. The position emphasizes autonomy, flexibility, and meaningful patient outcomes, with opportunities to incorporate telehealth and virtual care for enhanced access and continuity.

Responsibilities

• Perform evaluations and develop individualized treatment plans.
• Deliver evidence-based therapy to support patients' mobility, independence, and wellness.
• Document care accurately within EMR.
• Collaborate with interdisciplinary teams.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Include telehealth options to extend care flexibility.

Qualifications

• Valid Physical Therapy license.
• Graduation from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Physical demands

• Lifting, repositioning, and assisting with transfers.
• Driving between locations.
