About the role
Seeking licensed Physical Therapists to deliver personalized, one-on-one care to older adults in their homes or senior living communities, utilizing the Geriatric House Calls™ model. The position emphasizes autonomy, flexibility, and meaningful patient outcomes, with opportunities to incorporate telehealth and virtual care for enhanced access and continuity.
Responsibilities
- Perform evaluations and develop individualized treatment plans.
- Deliver evidence-based therapy to support patients' mobility, independence, and wellness.
- Document care accurately within EMR.
- Collaborate with interdisciplinary teams.
- Educate patients, caregivers, and staff on safety and aging-in-place strategies.
- Include telehealth options to extend care flexibility.
Qualifications
- Valid Physical Therapy license.
- Graduation from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
Physical demands
- Lifting, repositioning, and assisting with transfers.
- Driving between locations.
