Physical Therapist

About the role

The Pavilion Rehabilitation and Nursing Center is seeking a compassionate Physical Therapist (PT), PRN to provide care to residents as directed by licensed nurses and as outlined in the plan of care.

Responsibilities

• Communicate resident progress or problems to supervisor and other team members.
• Evaluate, develop an appropriate skilled plan of care, and communicate with physicians as a registered therapist; provide treatments following the plan of care.
• Document daily resident participation with skilled treatment sessions in accordance with regulatory, licensing, payer, and accrediting requirements.
• Instruct resident's family or nursing staff on current functional level as indicated.
• Maintain equipment and work area in a safe and clean condition.
• Provide support to the center's interdisciplinary departments as directed by the Director of Rehab to ensure the highest level of care in the industry.

Requirements

• Graduate of an accredited Physical Therapy Program.
• Current and in good standing discipline-specific state license.
• One (1) year of experience in a Skilled Nursing Facility and/or Long-Term Care preferred.

Pay

$65.00/hour plus $3.00 weekend shift differential.
