About the role
Licensed Physical Therapist providing personalized in-home and senior living care to older adults, supporting mobility, independence, and quality of life.
Key Responsibilities
- Deliver one-on-one physical therapy in private residences and senior communities
- Perform evaluations, develop individualized treatment plans, and provide evidence-based therapy
- Support patient wellness, mobility, and functional independence through therapy sessions
- Document care accurately within the electronic medical record system
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and staff on safety and aging in place strategies
- Utilize telehealth and virtual care to enhance access and flexibility
Qualifications & Skills
- Valid Physical Therapy license or eligibility in practice state(s)
- Degree from an accredited Physical Therapy program
- Proficiency in computer use and electronic medical records
- Current CPR certification
Physical Requirements
- Ability to exert medium effort, assist with patient transfers, perform CPR, and drive between locations while wearing PPE when necessary
