Physical Therapist

About the Role

Remedy Home Health Care is seeking a licensed Physical Therapist (PT) for the East LA Territory (Montebello, Downey, Bellflower). This Per Diem / PRN position provides flexibility and autonomy while delivering high‑quality rehabilitation and community‑based care directly to patients in their homes, assisted‑living facilities, and senior‑care settings.

Responsibilities

• Conduct in‑home physical therapy evaluations and reassessments.
• Develop and implement personalized rehabilitation care plans focused on fall prevention, strength building, mobility training, and functional independence.
• Provide therapy services in home health, assisted‑living, and community care environments.
• Educate patients and families on safe mobility techniques and fall‑prevention strategies.
• Document visits using the WellSky (Kinnser) EMR system.
• Collaborate with interdisciplinary home‑health teams, including nurses, case managers, and physicians.

Qualifications

• Valid California Physical Therapist (PT) license (required).
• CPR certification (AHA approved).
• Minimum one year of clinical experience; home‑health experience preferred.
• Experience working with senior, geriatric, or community‑based populations is a plus.
• Strong clinical, communication, and patient‑education skills.
• Valid driver’s license, reliable transportation, and auto insurance.

Pay

Pay‑per‑visit: $120 USD – $150 USD, plus mileage reimbursement.
