About the role

Remedy Home Health Care is seeking a licensed Physical Therapist (PT) to join its growing Home Health team in Los Angeles County, serving Huntington Park, Maywood, South Gate and surrounding areas. This Per Diem / PRN position offers part‑time or full‑time flexibility, allowing you to provide high‑quality rehabilitation and community‑based care directly to patients in their homes, assisted living facilities, and other community settings. You will work primarily with senior and geriatric populations recovering from illness, injury, or surgery, helping them regain independence and improve quality of life.

Responsibilities

• Conduct in‑home physical therapy evaluations and reassessments
• Develop and implement personalized rehabilitation care plans
• Improve strength, balance, mobility, and functional outcomes
• Provide therapy in home health, assisted living, and community care settings
• Educate patients and families on fall prevention and safe mobility techniques
• Document visits using WellSky (Kinnser) EMR
• Collaborate with interdisciplinary home health teams

Requirements

• Licensed Physical Therapist (PT) – California (required)
• CPR certification (AHA approved)
• Minimum 1 year of clinical experience (home health preferred)
• Valid driver’s license, reliable transportation, and auto insurance

Qualifications

• Experience in senior care, geriatrics, rehabilitation, or community‑based care preferred
• Strong clinical, communication, and patient education skills
