About the role

Absolute Healthcare Resources is a provider of home health care services serving the cities and counties within and surrounding Fairfax, Virginia. Our goal is for the client (your loved one, your patient or you) to stay in the comforts of home while receiving the care from our nurses, therapists, case managers and home health aides.

Responsibilities

• Visit patients in their homes for one-on-one visits
• Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients
• Assess patients’ conditions and chart observations, and perform evaluation tasks including vital signs and medication review
• Administer medication as prescribed by the patient’s physician
• Provide education on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient’s quality of life
• Communicate effectively with patients/clients, their family members, team members, and other health care professionals

Requirements

• Graduate of an accredited Physical Therapy Program
• Current state license as a Physical Therapist
• Proof of current CPR certification
• Career-oriented, compassionate, and friendly demeanor
• Excellent communication skills
• Current driver’s license and reliable transportation
• Ability to work a flexible schedule and travel locally
