Physical Therapist

Duties Conduct thorough assessments of patients’ physical, cognitive, and emotional needs using physiology knowledge and medical terminologyDevelop and implement tailored care plans that address individual goals for activities of daily living (ADLs), mobility, sensory processing, and functional skillsCollaborate with PT, PTA, and COTA on staff. Manage discharge planning by evaluating patient progressProvide case management services for patients in our outpatient clinicEducate patients and caregivers on adaptive techniques and equipment to support ongoing therapy goalsMaintain detailed documentation of patient progress, treatment sessions, and care plans in compliance with healthcare standards Requirements Valid state licensure as an Physical Therapist with current registration or certification as required by local regulationsProven experience working in outpatient clinicsKnowledge of anatomy and physiology relevant to occupational therapy practices; familiarity with medical terminology essentialSkilled in developing care plans that incorporate patient-centered goals and discharge strategiesStrong case management skills with the ability to coordinate services across multiple providers and settingsExcellent communication skills to effectively collaborate with healthcare teams and educate patients/caregiversPelvic health experience is not required but strongly preferred Benefits Referral program Work Location Hybrid Compensation Range: $70 - $80
