About the role
We are seeking licensed Physical Therapists to deliver personalized, one-on-one care to older adults in private homes and senior living communities, utilizing a flexible schedule and a signature Geriatric House Calls™ model.
Responsibilities
- Provide evidence-based physical therapy to enhance mobility, independence, and overall well-being.
- Perform evaluations, develop individualized care plans, and deliver therapy sessions.
- Document patient progress accurately within the EMR system, adhering to regulatory standards.
- Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.
- Educate patients, caregivers, and staff on safety and aging-in-place strategies.
- Incorporate telehealth and virtual care to increase access and flexibility.
Qualifications
- Valid Physical Therapy license and degree from an accredited program.
- Proficiency with computer systems and EMR documentation.
- Current CPR certification.
- Ability to meet physical demands such as lifting, repositioning patients, and driving.
