Licensed Physical Therapist providing personalized, in-home and virtual care to older adults in private residences and senior living communities.
Responsibilities
- Conduct evaluations
- Develop individualized treatment plans
- Deliver evidence-based therapy to improve mobility and independence
- Document in EMR
- Collaborate with interdisciplinary teams
- Educate patients and caregivers
- Utilize telehealth when appropriate
Qualifications
- Valid PT license
- Degree from accredited program
- Computer and EMR proficiency
- CPR certification
- Ability to meet physical demands such as lifting and assisting with transfers
